Explanation:  Multiple volumes with the same name were found. One of them was discarded from memory. The data is still intact on disk.

Action:  Because each volume on a given server must have a unique name, take one of the physical disks offline. You might have to delete one of the volumes and then bring the disks online simultaneously.
